MyMedicare registration.

MyMedicare is a voluntary patient registration model introduced by the Australian Government in October 2023 through Services Australia. It formalises the relationship between you and the practice you consider your regular place of care.

Registration is free, and you can end it at any time. It does not change your Medicare card or how your existing Medicare rebates work. It sits alongside, not instead of, your usual Medicare arrangements.

You can only be registered with one practice at a time. If you attend more than one SMSG centre, choose the centre you see most often as your MyMedicare registration.

Registering with SMSG through MyMedicare supports the kind of care that benefits from continuity: chronic condition management, long-term prescriptions, and coordinated review across specialties. Your registered practice becomes your central point of clinical coordination.

MyMedicare registration also unlocks additional Medicare services that are only available to registered patients, including longer telehealth consultations for eligible complex needs and the enhanced Chronic Disease Management Plan structure introduced under recent reforms.

Registration is completed online per centre. Choose the SMSG centre you attend most often, and complete the short Automed-integrated form.

• Your Medicare card number and reference number (the digit to the right of your name on the card).
• Full name, date of birth, and contact details.
• Your myGov account details if you plan to complete the linked steps through myGov (optional; the SMSG-side form does not require myGov).
• Confirmation that you consider the SMSG centre you are registering at as your regular practice.

Registering does not restrict where you can be seen. You can still attend any GP or specialist, at SMSG or elsewhere. What registration signals to Medicare is which practice provides your ongoing continuity of care.

Practically, once you are registered:

• Longer telehealth consultations become available where clinically appropriate.
• Certain Medicare items for chronic disease management and coordination become claimable.
• Your registered practice can more easily coordinate care across the SMSG group and with external services.

Registration is voluntary and can be ended or transferred at any time, either through Services Australia or by letting reception know.

If you are already an established patient at Earlwood, Bangor or Sans Souci, you can register with MyMedicare at any time using the link for your usual centre. Being registered as a new patient at SMSG and being registered with SMSG through MyMedicare are two separate steps.
